Who this is for. Adults 18 and over who are physically located in Florida at the time of each visit. This practice isn't the right fit if you need emergency care, a higher level of care (such as hospitalization or intensive outpatient treatment), or treatment primarily for substance use. If that's you, I'll help point you to the right place.
Billing. The evaluation is paid at booking. Memberships are billed monthly on the date you enroll, by card, HSA, or FSA. There's no long-term contract.
Cancelling. Cancel any time with notice before your next billing date. If you leave, I'll make sure you have enough medication and a plan for who takes over your care.
Missed appointments. Please give 24 hours' notice to reschedule. Visits missed without notice count as used.
Insurance. I don't bill insurance, but you'll get a superbill each month you can submit for out-of-network reimbursement. Insurers reimburse the visits themselves, not the membership's messaging or access.
Good Faith Estimate. Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges before care begins. I'll send one with your welcome packet, and you can ask for one any time.
Controlled medications. I prescribe stimulants and other controlled medications when clinically appropriate and permitted under current Florida and federal telehealth rules. This requires a full evaluation, prior records when available, a check of Florida's prescription monitoring database, and regular follow-up.
Privacy. Visits and messages use HIPAA-compliant, encrypted platforms. Please don't include personal health details in the form on this page; we'll cover them privately.
Have these ready for your evaluation:
- A photo ID
- A list of current medications and doses
- Names of medications you've tried before, if you remember them
- Contact information for your therapist or primary care doctor (optional)
- Your insurance card, if you plan to seek reimbursement
